Duty of Candour and what it means for One to One clients

Purely and simply the woman is at the very centre of everything that we do. Every decision, whether clinical or business, puts the you and your family first as endorsed in our core values below. If something goes wrong with your care, we are open and honest about it. We include and support our clients in all aspects of care and this includes when things go wrong. You and your family will be included in the incident review process. Learning will be shared across One to One and partner organisations.

How will you be involved?
Our risk management team will conduct a full investigation and will keep in contact with you throughout.
For a clinical incident, you will be invited to the investigation meeting .
You will receive a copy of the final report together with a letter inviting you to a debrief session in your home or our office wherever you choose.
For Complaints – you will receive the final report within 20 working days

For Clinical Incidents - you will receive the report with 60 working days
When an incident occurs or a complaint received
Staff will notify a senior staff member straightaway
You and the staff caring for you will be provided with immediate support and assistance
You and the staff caring for you will be provided with immediate support and assistance
A full and truthful account of the incident and everyone involved will be written down and shared with you.
Within 10 working days
Our risk management team will conduct a full investigation and will keep in contact with you throughout.
The team will provide a main contact for you and your family to keep in contact while the incident/complaint is investigate
You will be offered ongoing support
We will encourage you to email, phone or write to us with your recollection of events
How will you be involved ?
We share learning from any mistakes with all our staff to make improvements to our service.
We develop action plans and make sure this is carried out
We add action plans to ongoing training to ensure we minimise recurrence of any mistakes
